What this trial studies

This initiative aims to empower Latino(a) church members to become Community Health Workers (CHWs) who will be trained in diabetes management and health access issues. By focusing on social determinants of health, the program seeks to address barriers to healthcare quality and access within the Latino community. Participants will be engaged through their faith-based institutions, which serve as accessible venues for health outreach. The effectiveness of this intervention will be tracked using an online platform to measure improvements in healthcare access and quality.

Who should consider this trial

Good fit: Ideal candidates for this initiative are Spanish-speaking Latino(a) adults who are at risk for or diagnosed with Type 2 diabetes.

Not a fit: Patients who are pregnant, have Type 1 diabetes, are not Spanish-speaking, or have severe mental health conditions may not benefit from this initiative.

Eligibility criteria

Inclusion Criteria:

* Spanish-speaking Latino(a)s adults

Exclusion Criteria:

* pregnant state or anticipated state in the next 6 months
* Type 1 diabetes diagnosis
* Not Spanish-speaking, and
* Self-disclosed diagnosis of schizophrenia, psychotic/delusional disorder, or severe Alzheimer's disease.
