What this trial studies

This is a Type II hybrid effectiveness–implementation cluster randomized trial conducted in 12 basic health facilities across Namobuddha Municipality using the RE-AIM framework. Clusters are randomized to an intervention where female community health volunteers (FCHVs) and frontline health workers add diabetes screening, lifestyle counseling, group care, and referral support to existing hypertension activities, versus a control arm receiving standard facility-based care per Ministry of Health guidelines. The trial leverages an existing cohort of about 520 patients enrolled in FCHV-led hypertension programs to follow clinical and implementation outcomes. Investigators will measure reach, adoption, fidelity, and patient-level changes in blood pressure and HbA1c to judge feasibility and effectiveness.

Who should consider this trial

Good fit: Adults aged 30 or older with hypertension (BP ≥140/90 mmHg or on antihypertensive medication) and/or diabetes (HbA1c ≥6.5% or on diabetes medication) who can provide informed consent are eligible.

Not a fit: People with disabilities that limit participation, those with severe illness requiring bed rest, pregnant women, or individuals not served by the municipal health network are unlikely to benefit from this community-based program.

Eligibility criteria

Inclusion Criteria:

* 30 years or older
* have a high blood pressure of 140/90 mmHg or under hypertension medication
* have a high blood sugar of 6.5% or more (hba1c) or under diabetes medication are able to provide informed consent

Exclusion Criteria:

* any form of disabilities limiting participation in the study
* severe illness requiring bed rest, and
* pregnant women, due to their special health needs.
