CLUBE: UBE lumbar stability checked with 3-month dynamic X‑rays
Assessment of Spinal Stability by Dynamic Radiography After Unilateral Biportal Endoscopic Lumbar Decompression Surgery
This study will test whether UBE (endoscopic) lumbar decompression keeps the spine stable at 3 months using dynamic X‑rays in people aged 50 and older who need one- or two-level surgery.

What this trial studies
Patients aged 50 or older who require single- or two-level lumbar decompression using uniportal/bilateral endoscopic (UBE) techniques are enrolled at a single center. All participants must have a preoperative dynamic radiograph within 3 months of surgery and will receive an additional dynamic radiograph 3 months after surgery. Spinal stability is defined by the absence of abnormal sagittal translation (≥3 mm) between flexion and extension and will be measured on the 3-month dynamic X‑ray. Patients with preoperative mobile spondylolisthesis (≥3 mm), prior lumbar fusion, or inability to attend follow-up are excluded.
Who should consider this trial
Good fit: People aged 50 or older scheduled for single- or two-level lumbar decompression with UBE who had a preoperative dynamic X‑ray within 3 months and can attend the 3‑month follow-up are ideal candidates.
Not a fit: Patients with a preoperative mobile spondylolisthesis (≥3 mm), a prior lumbar arthrodesis, or those who cannot return for the 3‑month radiograph are unlikely to benefit from this protocol.
Why it matters
Potential benefit: If successful, the approach could reduce the need for fusion surgery, helping more patients keep spinal motion and potentially recover faster.
How similar studies have performed: Prior reports of endoscopic lumbar decompression show less muscle injury and quicker recovery than open surgery, but high-quality evidence specifically on postoperative spinal stability is limited, making this a relatively novel focus.
