This project will test whether analyzing clot tissue from people who had an endovascular thrombectomy for acute ischemic stroke can identify the stroke cause and help predict future vascular events.

What this trial studies
Patients who undergo endovascular thrombectomy for acute ischemic stroke will be asked for consent to collect their removed clot material. Enrolled patients receive standard diagnostic workup and best medical treatment while clot samples undergo RNA expression profiling and histological characterization. Researchers will classify stroke mechanism (atherosclerotic, cardiac embolism, other, or unknown) using clinical data and clot features. Participants will be followed in outpatient clinic for up to two years to monitor recurrent stroke, death, and bleeding events while clot data are correlated with outcomes.
Who should consider this trial
Good fit: Adults with acute ischemic stroke who have undergone endovascular thrombectomy and can provide informed consent are the intended participants.
Not a fit: Patients who do not undergo thrombectomy, who cannot provide usable clot samples, or who are unable to attend follow-up visits are unlikely to benefit from this approach.
Why it matters
Potential benefit: If successful, clot analysis could help clinicians identify the underlying cause of a stroke more accurately and better predict who is at higher risk for future vascular events, allowing more targeted prevention.
How similar studies have performed: Previous histological and molecular studies of retrieved clots have shown promise linking clot features to stroke cause, but RNA-based classification and long-term prognostic use remain relatively novel and under active study.
