We will see if clindamycin works as well as other antibiotics for adults with Streptococcus periprosthetic joint infections.

What this trial studies

This is an observational, retrospective comparison of outcomes for adults treated for Streptococcus spp. periprosthetic joint infections at participating centers between 2013 and 2024. Patients meeting EBJIS criteria for probable or proven infection who underwent surgery are grouped by whether they received clindamycin or other antibiotic regimens, and treatment success rates are compared. Clindamycin is of interest because it has high oral bioavailability and good bone and joint penetration and is used in France as an alternative to amoxicillin for some Streptococcus osteoarticular infections. Patients who had joint resection/amputation or were on preoperative suppressive antibiotics (>6 months) are excluded from the analysis.

Who should consider this trial

Good fit: Adults (18+) operated for probable or proven Streptococcus spp. periprosthetic joint infection at the participating center(s) between 2013 and 2024 who were not on long-term suppressive antibiotics and did not undergo joint resection or amputation.

Not a fit: Patients who had joint resection or amputation, those on preoperative suppressive antibiotic therapy (>6 months), or infections not caused by Streptococcus spp. would not be expected to benefit from this comparison.

Why it matters

Potential benefit: If clindamycin is shown to be as effective, it could provide an effective oral option with good bone penetration that may simplify treatment and help preserve implants.

How similar studies have performed: Clindamycin has established use and reported effectiveness for other Streptococcus bone and joint infections in France, but its specific performance for periprosthetic Streptococcus infections is less well documented.

Inclusion Criteria:

* Adult patient (aged 18 years or older) at the time of surgery for infection
* Probable or proven periprosthetic infection with Streptococcus spp. according to EBJIS diagnostic criteria
* Patient operated on in one of the associated centers between 2013 and 2024

Exclusion Criteria:

* Patient who underwent joint resection or amputation
* Patient who was treated with suppressive antibiotic therapy (defined as antibiotic treatment lasting more than 6 months) which was decided upon prior to surgery
