What this trial studies
This randomized trial will enroll 480 African American adults aged 55+ in the Greater Houston area and follow them for 24 months. Participants are randomly assigned to pharmacist-only medication management or to a combined pharmacist plus community health worker (CHW) intervention that also offers personalized support and group workshops. Both arms receive a digital home blood pressure monitor and guidance on home monitoring. The trial measures blood pressure control and related clinical and behavioral outcomes over two years.
Who should consider this trial
Good fit: African American adults aged 55 or older living in Greater Houston with a history of hypertension who take at least one antihypertensive medication, can use a smartphone, and are able to check blood pressure at home.
Not a fit: People with significant cognitive impairment who cannot self-monitor blood pressure, those unable to check BP at home, those planning to relocate outside Houston within a year, or those who cannot communicate in English are unlikely to benefit from participation.
Why it matters
Potential benefit: If successful, the combined pharmacist and CHW approach could improve blood pressure control and lower the risk of heart attack, stroke, and other complications in older African American adults.
How similar studies have performed: Previous pharmacist-led medication management and CHW programs have shown promise for improving blood pressure in underserved groups, but long-term randomized trials combining both approaches remain limited.

Inclusion Criteria: Individuals are eligible to the study if they are: * 55 years or older * Have at least 2 high blood pressure readings ≥ 130/80 mmHg within one month * Past Medical History of hypertension \> 1 year * Taking at least 1 antihypertensive medication * Independent Activities of Daily Livings (ADL) and instrumental Activities of Daily Livings (iADL) * Have a SmartPhone * Live within the Greater Houston Metropolitan Areas * Able to speak English Exclusion criteria: * Unable to verbalize comprehension of study or impaired cognitive function (e.g. dementia) * Not able to monitor blood pressure at home * Plans to relocate outside Houston within the next year
