What this trial studies
This mixed-methods evaluation examines how a bundle of caregiver support initiatives at Tan Tock Seng Hospital is delivered and whether caregivers and their care recipients benefit. Eligible caregivers complete three surveys (at hospital admission, around discharge, and one month after discharge) and may take part in an optional interview. The study combines process evaluation—documenting delivery, reach, and implementation—with outcome measures of caregiver stress, burden, and resilience as well as patient-related outcomes. Findings will identify which components of the support bundle are most useful and how they achieve impact.
Who should consider this trial
Good fit: Ideal participants are caregivers aged 21 or older who are the main or secondary caregiver of an older person receiving home-based care, can read and speak English, and use a smartphone.
Not a fit: Caregivers who are cognitively impaired, unwilling to use the required app, unable to use English comfortably, or who do not engage with the offered services are unlikely to benefit.
Why it matters
Potential benefit: If successful, the initiative could reduce caregiver stress and burden, strengthen caregiver resilience, and indirectly improve outcomes for older patients.
How similar studies have performed: Previous caregiver support and digital interventions have shown modest improvements in caregiver burden and coping, but bundled hospital-based implementations are less well characterized.

Inclusion Criteria: * 21 years of age and above * Main or secondary caregiver of an older person receiving home-based care * Able to read and converse in English * Has a mobile smartphone and is able to navigate basic functionalities on mobile apps Exclusion Criteria: * Participants who are cognitively impaired * Unwilling to download and utilise app * Unable to read or converse in English comfortably
