This trial will try adding daily Buteyko breathing exercises to usual care to see if it helps seniors with chronic Eustachian tube dysfunction.

What this trial studies

Forty seniors with unilateral, chronic obstructive Eustachian tube dysfunction lasting more than three months will be randomized into two groups of 20. Both groups will receive usual care consisting of budesonide nasal spray (64 micrograms once daily) plus a tablet twice daily containing pseudoephedrine 20 mg and loratadine 5 mg for two weeks. One group will additionally perform 30-minute Buteyko breathing sessions twice daily for 14 weeks. Key exclusions include obesity and significant cardiac or respiratory disease, and the trial is conducted at Cairo University in Dokki, Giza.

Who should consider this trial

Good fit: Ideal candidates are seniors with unilateral chronic obstructive Eustachian tube dysfunction persisting longer than three months who do not have obesity or major cardiac or respiratory disease.

Not a fit: Patients with obesity, significant cardiac or respiratory conditions, bilateral or non-obstructive Eustachian dysfunction, or other contraindications are unlikely to benefit or may be excluded.

Why it matters

Potential benefit: If successful, adding Buteyko breathing could reduce ear pressure symptoms and improve Eustachian tube function without more medications.

How similar studies have performed: Buteyko breathing has some use in other airway conditions, but its application to Eustachian tube dysfunction is novel with limited prior evidence.

* Eustachian Tube Dysfunction (chronic obstructive form)
* unilateral ear affetion
* senior patients
* chronci affection more than 3 months

Exclusion Criteria:

* obesity
* cardiac probelms repsiratory problems
