This trial will try whether a buccal nerve trunk block or the conventional buccal nerve block gives better pain control and reduces extra injections for adults having lower molar extractions.

What this trial studies

This randomized interventional trial compares the conventional buccal nerve block with a buccal nerve trunk block for anesthesia of the buccal mucosa during extraction of mandibular molars. Pain will be measured with a visual analogue scale (VAS) during both the injection and the extraction, with VAS scores over 4 labelled as positive pain perception. Need for supplemental infiltration is defined as use of more than one anesthesia cartridge and will be recorded as a primary outcome alongside intraoperative pain. The trial is conducted at a single center and enrolls adults aged 20–60 undergoing mandibular molar extraction.

Who should consider this trial

Good fit: Ideal candidates are adults aged 20–60 of either sex who require extraction of mandibular molars and do not have exclusions such as pregnancy, associated tooth pathology, ongoing radiotherapy/chemotherapy, or mandibular fracture.

Not a fit: Patients who are pregnant, have tooth-related pathologies (cysts or tumors), are receiving radiotherapy or chemotherapy, have a mandibular fracture, or are outside the 20–60 age range are unlikely to be eligible or to benefit.

Why it matters

Potential benefit: If successful, the buccal nerve trunk block could reduce pain during injection and extraction and lower the need for additional anesthesia injections.

How similar studies have performed: Smaller clinical and anatomical reports suggest the buccal nerve trunk block can improve buccal mucosa anesthesia compared with conventional techniques, but high-quality randomized data are limited.

Inclusion Criteria:

* Patients aged 20 to 60 years
* Patients of both gender
* Patient requiring extraction of mandibular molars

Exclusion Criteria:

* Pregnant women
* Pathologies associated with teeth i.e. cysts, tumors
* Patients on radiotherapy or chemotherapy
* Patients with mandibular fracture
