What this trial studies
This interventional project at Duke targets adults with rheumatoid arthritis and uncontrolled hypertension who receive both primary care and rheumatology care within the Duke Health System. Participants will join a blood-pressure intervention arm designed to identify barriers to hypertension care and provide targeted support to improve control. The team will track blood pressure measurements and care processes over time to see whether the intervention improves management. The ultimate aim is to close care gaps that contribute to higher heart disease risk in people with RA.
Who should consider this trial
Good fit: Adults (18+) with a history of rheumatoid arthritis, uncontrolled hypertension, and who receive both primary care and rheumatology care within the Duke Health System are ideal candidates.
Not a fit: People without rheumatoid arthritis, those whose blood pressure is already controlled, or patients who do not receive both primary and rheumatology care at Duke are unlikely to benefit from this project.
Why it matters
Potential benefit: If successful, the program could help people with RA get better blood pressure control and lower their future risk of coronary artery disease.
How similar studies have performed: Hypertension-control programs in general populations have improved blood pressure, but interventions specifically targeting hypertension care barriers in rheumatoid arthritis patients have been less widely tested.

Inclusion Criteria: * 18 years old or older * Uncontrolled hypertension * History of rheumatoid arthritis * Receive both primary care and rheumatology care from Duke Health System Exclusion Criteria: * Age less than 18 years old * Healthy volunteers without rheumatoid arthritis and hypertension * Do not receive both primary care and rheumatology care from Duke Health System * Cognitive impairment with lack the capacity to consent to study participation * Pregnant women * Prisoners
