We will test whether adding analytical stretching to an Alfredson exercise program plus health education improves ATOR screening scores over 12 weeks in senior club runners with Achilles tendinopathy.

What this trial studies
Senior club runners with Achilles tendinopathy who train regularly will be enrolled and receive either the Alfredson eccentric exercise program plus health education or that same program with additional analytical stretching, delivered at a physiotherapy clinic in Zaragoza. Change in ATOR screening test scores will be measured at baseline and after 12 weeks to capture symptomatic and functional changes. Participants with recent invasive physiotherapy, recent fluoroquinolone use, autoimmune disease, or limited comprehension are excluded. The comparison aims to determine whether adding analytical stretching provides greater improvement than exercise and education alone.
Who should consider this trial
Good fit: Ideal candidates are senior runners who belong to a running club, have completed at least five 10-km races in the past year, train at least three times per week, and have Achilles tendinopathy.
Not a fit: Runners with autoimmune disease, recent invasive physiotherapy, recent fluoroquinolone use, or limited ability to complete surveys may not be eligible or may not receive benefit from participation.
Why it matters
Potential benefit: If successful, the combined program could produce faster or greater improvement in pain and tendon function for older runners with Achilles tendinopathy.
How similar studies have performed: Eccentric loading protocols such as the Alfredson program have shown benefit in Achilles tendinopathy, whereas the added value of analytical stretching is less well studied.

Inclusion Criteria: * Senior runners * They belong to a running club * That they have run at least 5 10-kilometer races in the past year * They should work out at least three times a week Exclusion Criteria: * Runners who have received physiotherapy treatment involving invasive techniques in the last 3 months * Runners who have received pharmacological treatment with fluoroquinolones (levofloxacin, ciprofloxacin) in the last year * Runners with autoimmune diseases * Individuals with limited comprehension who may be unable to answer surveys
