What this trial studies
The Inpatient Nursing Frailty Assessment (INFA) program is designed to improve care for frail older adults admitted to hospitals without geriatrician oversight. It utilizes a Comprehensive Geriatric Assessment (CGA) approach, where nurses collect data on various health domains during routine admissions. This information helps identify health issues early and develop personalized care plans to mitigate risks of functional decline. The program also includes nurse-led interventions and improved referral processes to specialist care, ensuring comprehensive support for patients.
Who should consider this trial
Good fit: Ideal candidates for this program are hospitalized patients aged 65 and older with a Clinical Frailty Scale rating of 4-6.
Not a fit: Patients who are electively admitted or those coming from nursing or sheltered homes may not benefit from this program.
Why it matters
Potential benefit: If successful, this program could significantly enhance the quality of care and outcomes for frail older adults in hospitals.
How similar studies have performed: Other studies have shown success with similar Comprehensive Geriatric Assessment approaches, indicating potential for positive outcomes.
