What this trial studies

This study aims to develop and pilot test an AI-assisted, individualized, family-focused lifestyle modification intervention for glycemic control in individuals with Type 2 diabetes and physical disabilities. Participants will be randomly assigned to either an intervention group receiving telecoaching support or an attention-control group. The intervention will consist of weekly and bi-weekly telecoaching calls over six months, followed by an additional six months of follow-up without telecoaching. The goal is to assess the effectiveness of this approach in improving diabetes management.

Who should consider this trial

Good fit: Ideal candidates include individuals diagnosed with Type 2 diabetes who also have a permanent physical disability.

Not a fit: Patients currently enrolled in any diabetes-related intervention or those with severe untreated depression or other significant health issues may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* Diagnosis of Type 2 Diabetes Mellitus,
* Living with a permanent physical disability such as spinal cord injury (SCI), spina bifida, multiple sclerosis, stroke, etc.
* Ability to converse and read in English

Exclusion Criteria:

* Current enrollment in any diabetes-related intervention
* Severe untreated depression in the past six months
* Major cardiac event in the past twelve months
* Uncontrolled blood pressure
* Resting tachycardia
* Renal failure
* Severe peripheral neuropathy
* Unavailability of a smartphone
