This project will test whether Street Racket, a simple group racket game, can be added to outpatient pulmonary rehabilitation for adults with chronic lung disease to make sessions more acceptable and engaging.

What this trial studies

This interventional feasibility project embeds Street Racket sessions within the existing ambulatory pulmonary rehabilitation program at the University Hospital of Zurich. Participants rate feasibility and appropriateness at the first and last Street Racket session and record average and peak breathlessness during each session, while attendance and reasons for absence are tracked. The training content is the same for study participants and non-participants, with study involvement limited to additional brief questionnaires and routine data collection. Outcomes focus on acceptability, practical barriers, and session tolerability rather than clinical efficacy.

Who should consider this trial

Good fit: Adults over 18 who are prescribed and currently participating in ambulatory pulmonary rehabilitation at the University Hospital of Zurich and who can stand or walk for up to an hour (with short seated breaks as needed) are eligible.

Not a fit: People who require non-portable oxygen delivery systems, need walking aids for balance, or have major language, psychological, or orthopedic barriers to standing/walking are unlikely to benefit from participation in this protocol.

Why it matters

Potential benefit: If successful, Street Racket could increase enjoyment, attendance, and longer-term adherence to pulmonary rehabilitation programs and may improve mental wellbeing alongside physical activity.

How similar studies have performed: Group- and game-based exercise approaches have shown promise for improving adherence and mood in chronic lung disease, but the use of Street Racket specifically is novel with limited direct prior data.

Eligibility criteria

Inclusion Criteria:

* Any sex or gender
* Age \>18 years
* Prescribed APR by a physician and currently participating in APR at the USZ
* Physically and psychologically capable of following instructions and standing/walking for up to one hour (short, seated breaks are allowed if needed)

Exclusion Criteria:

* Require supplemental oxygen therapy that cannot be delivered via a portable backpack system (e.g., large oxygen cylinders on wheels, which pose a fall risk)
* Are unable to follow study procedures due to language barriers, psychological conditions, or orthopaedic limitations that prevent them from standing or walking for up to one hour
* Require walking aids due to impaired balance (e.g., walking sticks, rollator, etc.)
