This project will test an adapted hospital-to-home care program for adults living with HIV and a heart or metabolic condition to improve care after discharge in Lilongwe.

What this trial studies
The study will enroll 75 consecutive adults admitted to internal medicine at Kamuzu Central Hospital who are living with HIV and have at least one cardiometabolic condition, and deliver an adapted Transition of Care Model (TCM) according to a locally developed SOP. Using a mixed-methods approach, researchers will collect surveys and interviews to measure acceptability and feasibility of both inpatient and post-discharge components. The pilot will track key 3-month outcomes including mortality, readmission, retention in care, ART/NCD medication adherence, and dual control of HIV and NCDs. Pilot participant outcomes and mediating indicators will be compared with a comparable historical control group who received routine care at the hospital.
Who should consider this trial
Good fit: Adults (18+) living with HIV who are admitted to internal medicine at Kamuzu Central Hospital with at least one cardiometabolic condition and who reside in Lilongwe urban are ideal candidates.
Not a fit: People who live outside Lilongwe urban, children, or adults without comorbid cardiometabolic conditions are not eligible and are unlikely to benefit from this adapted TCM.
Why it matters
Potential benefit: If successful, the intervention could reduce post-discharge mortality and readmissions while improving medication adherence and retention in care for adults with HIV and NCDs in Lilongwe.
How similar studies have performed: The Transition of Care Model has demonstrated benefits in high-income settings such as the USA, but its application for post-discharge HIV and NCD management in Malawi is novel and largely untested.

Inclusion Criteria: * 18 years of age or older * living with HIV * admitted to internal medicine * has at least a cardiometabolic NCO as the primary or secondary reason for admission based on the HIV inpatient consultation Exclusion Criteria: * patients living beyond Lilongwe urban
