Inclusion Criteria: * aged 18 years and older * current smoker * English speaking * Patient at Mile Square Health Center (Main, Englewood, South Shore, Auburn Gresham, Cicero, Back of the Yards) Exclusion Criteria: * aged less than 18 * non-smoker * non English speaking * Not a patient at MSHC
