What this trial studies

This intervention focuses on self-management of hypertension among Native Hawaiian and Pacific Islander (NHPI) adults through a program called ENACTS. The program includes peer-facilitated education on healthy diets, medication adherence, and lifestyle changes, delivered over six months. Participants will be randomized into either the ENACTS intervention or usual care, with outcomes measured at various intervals using a mobile app for tracking. The study aims to evaluate changes in blood pressure, food purchasing behaviors, and family support for hypertension management.

Who should consider this trial

Good fit: Ideal candidates are NHPI adults aged 18 and older with a self-reported diagnosis of hypertension and elevated blood pressure.

Not a fit: Patients who are pregnant, undergoing dialysis, or receiving treatment for a terminal illness may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* Self-reported physician diagnosis of hypertension and/or type 2 diabetes confirmed by a prescription for an anti-hypertensive medication and/or type 2 diabetes medication;
* Self-reported NHPI race;
* \*Age 18 years and older at enrollment;
* Measured systolic BP ≥ 140 mmHg on enrollment; measured baseline fasting glucose \>125
* reliable internet access

* current email address
* \*Ability to understand written and spoken English; and
* \*Ability and willingness to follow all study protocols.

Exclusion Criteria:

* Pregnant or planning on becoming pregnant throughout the course of the study;
* Currently undergoing dialysis or treatment for a terminal illness;
