Understanding the transition of dementia patients from skilled nursing facilities to home
The Coming Home After Rehabilitation and Transition from a Skilled nursing facility with Dementia Study (The CHARTS-D Study)
This study looks at the difficulties people with Alzheimer's and similar conditions face when moving back home from nursing facilities, aiming to find out what causes them to end up back in the hospital, so we can make their care and support better for them and their families.

What this research studies
This research investigates the challenges faced by patients with Alzheimer's disease and related dementias as they transition from skilled nursing facilities back to their homes. It aims to identify factors that contribute to hospital readmissions and other adverse outcomes during this transition. By analyzing a large national dataset that includes social and economic information, the study seeks to understand how cognitive impairment and other variables affect patient outcomes. The findings could help improve post-hospitalization care and support for these patients and their caregivers.
Who could benefit from this research
Good fit: Ideal candidates for this research are patients diagnosed with Alzheimer's disease or related dementias who are transitioning from a skilled nursing facility to their home.
Not a fit: Patients who are not transitioning from a skilled nursing facility or those without a diagnosis of dementia may not benefit from this research.
Why it matters
Potential benefit: If successful, this research could lead to better care strategies that reduce hospital readmissions and improve the quality of life for dementia patients transitioning home.
How similar studies have performed: Previous research has shown success in improving transitions of care for patients with other conditions, but this specific focus on dementia transitions is relatively novel.
