This study is looking to help people who have survived lung or colorectal cancer by offering them friendly support through online sessions with experienced nurses, so they can better manage their health and improve their quality of life after treatment.

What this research studies
This research focuses on enhancing the care and quality of life for lung and colorectal cancer survivors by implementing a telehealth Survivor Self-Management (SSM) Intervention led by advanced-practice nurses. The program includes care coordination between oncology and primary care, personalized care plans, and skills training for managing health post-treatment. Participants will engage in five telehealth sessions and additional follow-up sessions to address their unique needs and improve their overall health outcomes. The study aims to address the significant disparities faced by these survivors in post-treatment care.
