What this research studies

This research focuses on enhancing the care and quality of life for lung and colorectal cancer survivors by implementing a telehealth Survivor Self-Management (SSM) Intervention led by advanced-practice nurses. The program includes care coordination between oncology and primary care, personalized care plans, and skills training for managing health post-treatment. Participants will engage in five telehealth sessions and additional follow-up sessions to address their unique needs and improve their overall health outcomes. The study aims to address the significant disparities faced by these survivors in post-treatment care.
